Give more help to caregivers who often ignore self-care: Experts – The Straits Times

Having been the main caregiver to her 87-year-old mother who has dementia for eight years, Ms Kris Foo's constant companion has been emotional, mental and financial stress.
The self-employed branding designer suffered from caregiver burnout in 2015, after facing daily bouts of tantrums that stemmed from her mother's depression and anxiety.
"I woke up one day and I just started crying. But looking back now, there were telltale signs - I was getting increasingly irritable and lost my focus on everything else besides caregiving," said Ms Foo, 53, who is single and has an older sister. Her mother has been divorced for 40 years.
"My business suffered as a result of this, and I had to start drawing on my mother's savings to cover her medical and household expenses," she added.
Ms Foo also grappled with negative feelings such as the loss of self-identity, suicidal thoughts and the harrowing reality that dementia patients will never "get better", as their condition worsens over time.
In 2018, Ms Foo began sending her mother to daycare - which gave her five extra hours each day to catch up on her work and sleep.
Seeing a counsellor also helped her to better identify and cope with stress, though she now finds herself grappling with the larger issue of self-care.
For instance, uncertainty over her future and retirement plans has weighed on her.
During a visit to a geriatrician in July, she was told that it was time to "care for the caregiver", as the doctor could tell she was heading towards another burnout.
Applications were made to send her mum to a nursing home and they are awaiting the outcome.
Although Ms Foo knew that such a decision would be crucial for her mental well-being, it is one that she made while ridden with guilt.
"There's always that sense of abandonment tied to nursing homes. It has not been easy warming her up to the idea of moving to some place else that isn't her own home, and for her to understand that I can't look after her by myself any more," she said.
While Ms Foo yearns to restart her own life, the guilt is not easy to deal with.
"The shift in reality and embracing a lifestyle of not living with my mother will be a huge adjustment for me, and counselling and social support will be important as I make these transitions in life," she added.
Caregiver guilt is a familiar feeling for many, as the deeply entrenched values of filial piety and duty often come at the expense of self-care, said experts here.
Dr Annabelle Chow, principal clinical psychologist at Annabelle Psychology, noted that as society's view of familial caregivers is centred on duty and obligation, those who need help are worried of being dismissed or shunned.
In addition, many caregivers tend to under-report feelings of stress because of the guilt that they face.
"Caregivers are often used to a constant routine of caring, and when they are not, they feel guilty for being away, and that they are not doing their job well or fulfilling their duty.
"This worsens if they decide to take time off to recuperate, where they may feel 'selfish' for doing so," she added.
Very few caregivers seek help for themselves at her clinic, Dr Chow noted. Rather, they often accompany those they are caring for, and would confide their difficulties with the psychologist in passing during a consultation.
Social entrepreneur and former Nominated Member of Parliament (NMP) Anthea Ong said it is common for many caregivers to de-prioritise their own emotional needs out of guilt.
"Some think that it is their responsibility as a family member, and that asking for support is too difficult.
"But it is not mutually exclusive to be filial and still look after our own well-being, so caregiver education is paramount in making sense of these ideas and feelings," she noted.
Assistance and resources for caregivers could be made available and more visible at all general practitioner and other clinics, along with caregiver support offices at polyclinics, suggested Ms Ong.
Peer support training and caregiving classes could also be held at community centres, she added.
The Ministry of Social and Family Development (MSF) said SG Enable, a disability service and support organisation, has worked with social service agencies and the community to organise peer-to-peer mentor training for caregivers of persons with disabilities.
According to five agencies and support groups which The Straits Times spoke to, caregiving stress has been on the rise during the Covid-19 pandemic, a result of work-from-home arrangements, a lack of caregiving reprieve, and financial stress from reduced incomes and loss of jobs.
Existing help that caregivers can tap includes an annual $200 Caregivers Training Grant administered by the Agency for Integrated Care (AIC) to offset the cost of enrolling for courses.
Such courses could help caregivers build their capabilities and reduce the level of stress that they face, agencies said.
Caregivers of disabled persons may also be eligible for levy concessions if they wish to hire a foreign domestic worker (FDW), and a Home Caregiving Grant, a $200 monthly cash payout to defray the costs of caregiving.
The grant can also be used for hiring a FDW, said the MSF.
A survey by the Association of Women for Action and Research (Aware) last year showed that caregivers of elderly patients who employ FDWs spent an average of almost $1,300 monthly, whereas the monthly expenses without an FDW were around $380, with the difference likely due to greater caregiving needs.
However, Aware noted that FDWs themselves suffer stress and burnout, due to factors such as overwork and inadequate training for eldercare jobs.
Agreeing, Ms Foo noted that some of her FDWs had quit as they felt that they lacked training and were not equipped to provide eldercare.
The $200 Home Caregiving Grant also covers just a fraction of expenses, and does not apply to medical expenses for care recipients, she added.
Respite care services should also be more affordable and accessible, she said. Ms Foo considered night respite care for her mum, who regularly wakes up six to eight times a night, but the cost of $100 to $150 each night is prohibitive in the long term.
One way to improve affordability would be to allow payment for such services through Medisave or Medifund, she said.
A Caregiver Support Grant that provides additional financial aid from the Government would be helpful, said Ms Shailey Hingorani, Aware's head of research and advocacy.
Ms Ong agreed and called for employers to offer more flexible work arrangements and support programmes centred on the well-being of staff who are caregivers.
But more pressing is the need for a societal shift towards valuing the work of caregivers "as no less essential" than that provided by healthcare workers and other front liners.
"More visibility, appreciation and recognition should be given to them for the critical role that they play in society," she added.
Caregiver burnout refers to a point where a caregiver reaches his or her threshold, leading to physical, emotional and mental exhaustion.
According to Dr Annabelle Chow, principal clinical psychologist at Annabelle Psychology, signs of burnout include the lack of energy even with sufficient rest, having trouble relaxing, insomnia or hypersomnia, and diminished interest in activities that one used to enjoy.
These signs persist nearly every day for two weeks or more.
Those experiencing burnout tend to be more irritable and are susceptible to developing a mental health condition.
To take care of one's mental health, Dr Chow has the following tips:
1 Accept that everyone has limits to the stress he or she can take, and give yourself the permission to rest, feel and cry.
2 Ask for help when you need it. This is not just limited to respite care, but can be for small things like getting help to buy groceries or taking time off to go for a walk. Small "wins" are better than holding out for a "big break".
3 Join a caregiver support group. Having a group of caregiving friends to share stories and tips with could make a lot of difference.
4 Try mindfulness exercises. Mindfulness is a form of meditation which focuses on being aware of what you are sensing and feeling in the moment, without judging yourself.
There are plenty of free exercises online, and these practices take only a few minutes each day.
5 Make some time for yourself. Recall the things you enjoyed doing to relax, and try to engage in these activities. If you still feel burnt out after taking time off, you should seek help from a mental health professional.

New Delhi: The women Self-Help Groups (SHGs) in Uttar Pradesh are setting new precedents of development and doing wonders, not just locally but globally. In a major milestone, the handicraft products made by them are set to be showcased in an exhibition in Japan.
Uttar Pradeshs Mirzapur city, in particular, will feature in Japan Carpet Fair as the latter will exhibit world-famous foot-mats and wall hangings made by women of Self-Help Group named Gareeb Prerana Swayam Sahayta Samuh in Khajuri village of the city.
The Rugs and mats weaved by the local artisans in Mirzapur have already grabbed accolade from across the globe for its excellence. The local weavers amalgamate new perspectives on traditional crafts with state-of-the-art design technology to create authentic designs and visual masterpieces that will be now be displayed on a global platform. The stunning work is infused with vibrant colours and soft lustre.
The organisers of the fair were so impressed by the talents of these women that they invited the entire group to Japan and gave them an opportunity to sell their products at the Japan Carpet Fair which will be held in 2021.

Mumbai: Union Minister of State for Education, Sanjay Dhotre launched a self-help website Bandhu for students of IIT Bombay today at 11 am in the presence of Director of IIT Bombay Prof. Subhasis Chaudhuri.
Appreciating the efforts of alumni of IIT Bombay, the Minister said, A good education institution is one in which every student feels welcomed and cared for, where a safe and stimulating learning environment exists, where a wide range of learning experiences are offered, and where good physical infrastructure and appropriate resources conducive to learning are available to all students. Students are the prime stakeholders in the education system. Vibrant campus life is essential for high-quality teaching-learning processes. Towards this end, students should be given plenty of opportunities for participation in sports, culture/arts clubs, eco-clubs, activity clubs, community service projects etc.

Politicians must recognise racism is a scourge that has to be cleansed – Stabroek News

Dear Editor,
Racism is caused by race labelling; it is a global problem that is used as the main tool by politicians to secure power and power bases in populations that are weighted down by demographic labels, regardless of the economic and social systems practiced.
Capitalism, Communism or Socialism are all undermined by this unhealthy social construct that feeds off of recognizing another human being as different, in an inferior way.
Alan Watts, a philosopher and writer on Buddhism, Taoism, and Hinduism said: Nothing exists independently, everything is a thing, only in relation to everything else, therefore there are no separate things, no real selves or souls or egos and trying to cling to these things is the cause of suffering, discord and frustration.
Imagine in our Guyana that free and fair elections were held in 2020 and Walter Rodneys Working Peoples Alliance had won the election; let us imagine how Walter Rodney would have governed, with the power bestowed on him by our constitution.
Then our leaders must stop imagining and govern like Dr. Walter Rodney would. Under these circumstances, Guyana would have tangible and meaningful inclusive governance and massive economic growth, regardless of which administration governs.
In the imaginings, we assume the philosophy of Walter still obtains when he was killed on June 13, 1980, and that his philosophy would be the bedrock and priority of the new administration.
Then one immediately gets a sense of how misdirection, wrong paths and racism dominates our political culture, as it has existed since PPP split in 1955.
From whichever demographic we are separated into, I dare say guiltiness will abound amongst our political leaders, as they know they have embraced an anti-Guyanese political culture.
Racism is a disease. Poverty is a disease. Our politicians need to recognize that the cleansing of these scourges must be addressed in a deliberative and focused way.
To rephrase the great philosopher Aim Csaire: Politicians and politics and its various isms must be harnessed into the service of people and not the other way around, and moreover, absolutely not in a discriminatory manner.

Photographer, aerobics teacher and Key West volunteer extraordinaire Roberta DePiero was just looking for a way to get a little exercise this summer after the coronavirus shut down the Keys.
Her usual job as an aerobics teacher was gone because the gyms were closed. Her other job selling ads for theater and event programs was shut down because there were no local events taking place.
So, DePiero, who followed her soon-to-be ex-finance to Key West in 1991, started riding her bicycle around town. Joined by a friend, their cycling jaunts got longer and more elaborate.
I started riding my bike just to get exercise. We went to all the tourist locations and took cheesy photographs, she said. Then we started doing scavenger hunts, looking for historical markers. We rode every street in Stock Island, Key Haven and Key West. It took about two months.
Those cheesy photos were posted on DePieros social media pages, leading her to start paying attention to what other local photographers were posting; recording how they were dealing with the town and its business community being closed to visitors. She got the idea of pulling the images together into a book documenting local response to the pandemic and reached out to friend and local photographer Carol Tedesco, with whom she had previously published a 2013 photo book of the Key West Cemetery, After Life: Images From the Key West Cemetery.
The women set a three-month timeline for photos taken between March 1 and June 1, and reached out to other local photographers to collaborate on what was supposed to be a 150-page book, entitled, Isolated Island, The Key West Covid-19 Spring of 2020.
But the images were so spectacular that it grew to 27 photographers and 352 pages, DePiero said. Its a time capsule. In 100 years, people can open this up.
The images range from people getting COVID-19 tests, to underwater shots to pictures of empty stores and a deserted Duval Street. Photographer Kyle Campbell got Federal Aviation Authority permission to use a drone, since Key West International Airport was essentially closed, and took a heavens-view look at an empty city and surrounding waters. Publicist Andy Newman took pictures of the checkpoint on U.S. 1 and a deserted highway. Tedesco, Rob ONeal and Larry Blackburn, all local photojournalists (Editors note: ONeal is the staff photographer for The Citizen), looked at daily life, including crowded food banks and police horses patrolling an empty beach.
But the cherry on the top of the beautiful images is that all proceeds are going to benefit Sister Season Fund, which has been giving grants to help out-of-work employees in the service sector survive the coronavirus shutdown. With a limited edition of 1,000 books, priced at $75, Isolated Island will raise more than $38,000 for Sister Season if all are sold. As of this week, there are only 300 left and can be purchased at http://www.sisterseason.com.
Julie Hanson, Sister Season president, said the non-profit organization has given $1,000 grants to 339 applicants since COVID-19 hit Key West, draining its funds.
Our grants were running out, she said. We had fundraisers, online auctions. We sold a lot of masks. The community really stepped up to help us. Carol and Roberta were two of those people. Were so grateful for that.
Its amazing that my little, tiny idea blossomed into this, DePiero said. This is the biggest thing Ive ever done.
And that is saying something. DePiero has been an active volunteer for a wide range of local, state and national organizations since arriving in Key West. One of her early jobs was executive assistant, then catering manager for the former Holiday Inn La Concha. While there, she was given the task of being the hotels team captain for the new Relay for Life, the annual race to raise money for the American Cancer Society. DePiero eventually became chair of the event, then chair of the state Relay for Life Task Force, a member of the national taskforce, and local chair of the American Cancer Society. She is also past president of the Zonta Club of Key West, helped organize the Mote Marine Ocean Fest for its first six years, and helped start the Anne McKee Artist Fund of the Florida Keys, which raises money for local artists through an annual art auction that gives half the proceeds to the contributing artist, then uses the other half to award grants up to $2,000 to different artists in literary, visual and performing arts fields.
As for how she thinks Key West will come out of the coronavirus pandemic, DePiero feels positive about the recovery.
I personally feel weve dealt with it very well. Its made all of us living here stronger, like after [Hurricane] Wilma, she said. We may be recovering differently but maybe well get a different kind of people coming to town. I think well be alright.

Fall is a perfect time to explore passion for Persimmons – The Columbian

Although they grow well in our climate, persimmons seem like an exotic fruit, not often found in fruit bowls alongside apples, oranges and bananas. As for me, Im simply mad about them. Id rather have a luscious, ripe persimmon than a bowl of ice cream, and thats really saying something, because ice cream is the pinnacle of human culinary creation. (Ill brook no argument on this point.)
You may have had an unfortunate, if not downright scarring, experience with a persimmon in the past, due to some varieties extremely high levels of mouth-drying tannins. A few tannins are nice, such as in a glass of red wine. A whopping great bucketful of tannins, such as youd encounter in an unripe hachiya persimmon, will cause you to feel as though someone had opened a direct portal between your mouth and the Sahara desert.
The heart-shaped, deep orange hachiyas are intensely flavorful, but only if eaten at the point of extreme ripeness, when their insides have practically liquefied in their shiny skins. You can speed the process along by storing them inside a paper bag. Your patience will be rewarded with a bite of heaven a jammy treat thats sort of a cross between pumpkin pie and a sun-warmed, juicy peach.
If you cant wait that long for your heavenly reward, purchase the tannin-free fuyu variety: squat, sunset-orange packages of ready-to-consume goodness, delicious in every stage from crisp to squishy, but probably best somewhere in the middle. Eat them like an apple, skin and all, or peel them, puree them and use them in everything from cookies to risotto.
Boil 2 cups of very ripe, pulpy persimmons with 3/4 to 1 cup granulated sugar, 2 tablespoons lemon juice and 1 to 2 tablespoons lemon zest. Reduce heat to low and simmer for 10 to 15 minutes, then turn off the heat and allow the jam to cool and set. Transfer to Mason jars or another airtight container and store in the fridge for 1 to 2 weeks or freezer for a year or so. Serve with buttered toast, scones, biscuits, pancakes, waffles, blintzes or crepes, but not necessarily all those things at once, unless youre having a particularly hard day, or its a Wednesday.
This moist persimmon bread was what got me hooked on persimmons. The baker, my aerobics instructor, may have fed it to me to ensure that Id keep taking her class, a vicious caloric cycle of consume-and-burn.
Preheat oven to 325 degrees and oil a loaf pan. In a bowl, combine 1 1/2 cups flour, 1/2 teaspoon salt, 1 teaspoon cinnamon or pumpkin pie spice with 1 cup of any combination of chopped walnuts, pecans, raisins, dates, dried apricots, crystallized ginger or chocolate chips.
In another bowl, combine 2 eggs, 3/4 cup white sugar and 1/2 cup vegetable oil. In third bowl, mix 1 teaspoon baking soda with 1 cup persimmon pulp, stirring until there are no lumps. I recommend extremely ripe hachiya persimmons, but you can use very soft fuyus. Either way, they should be squishy.
Full disclosure: I only had enough ripe persimmons for 3/4 cup. I tasted a half-ripe hachiya, and it was like eating cotton. I improvised and substituted pumpkin puree and 2 tablespoons peach jam. I suppose I made persumkineach bread.
Add pulp to the egg mixture, then fold in the flour mixture. Pour batter into pan and bake for 75 minutes or until a knife comes out clean. I baked mine for probably an hour and 40 minutes preposterously long! So quick bread is a misnomer, but the result, although dark brown around the edges, is so good I now need another aerobics class.
Im always looking for ways to eat more leafy greens. Just kidding, Im really looking for ways to eat more goat cheese. This beautiful, scrumptious salad, which I discovered at http://www.proportionalplate.com, is worth saving for a COVID-safe Thanksgiving with your immediate family and your neighbors waving through the window, or you could make this today, just for funsies. It veers into foodie territory anything with radicchio and pomegranate seeds is dangerously trendy but recipes are merely inspiration for ways to use things I already have.
Cut two ripe but not too soft fuyu persimmons into thin wedges. Mix with 1 cup crumbled goat cheese and 4 cups each shredded radicchio and escarole. (I didnt have escarole and I couldnt find it at the market, so I used baby arugula.)
Add the seeds from 1 pomegranate. Since seeding a pomegranate produces a similar enjoyment quotient to clipping an angry cats claws, you should feel quite justified in substituting dried cranberries or chopped dried cherries.

Sylvester Stallones Rocky days are over, but at age 74, he hasnt given up his vigorous training regimen. It often consists of three or four sets of a dozen weightlifting exercises. He also begins and ends each workout with 10 to 20 minutes of cardio.
Fortunately, recent research suggests you dont have to emulate Stallone to age well and stay fit. A study on physical fitness intensity and older adults found that those who regularly did high-intensity interval training didnt live any longer than those doing moderate-intensity continuous training or sticking to standard activity guidelines.
The study, published in The BMJ, involved 1,567 adults in their 70s. Participants in the HIIT group performed two sessions weekly of high-intensity interval training with four four-minute bursts of activity at about 90% of peak heart rate with three-minute rest periods in between. The other group did moderate intensity continuous training at about 70% of peak heart rate for 40 minutes twice a week. And the control group followed standard guidelines for physical activity (a minimum of 30 minutes moderate activity daily combining aerobics and strength training). After five years, the researchers didnt find a difference in cardiovascular health or in all-cause mortality between the control group and the combined HIIT and moderate-intensity continuous training groups.
So dont fret if you cant emulate Stallones workout! Turns out, once youre over 70, a sly not Sly approach to exercise, by integrating it into your daily routine, is an effective way to protect your cardiovascular health and promote longevity.
